Please provide full given names, all previous names and applicable personal identifiers. You will be required to provide proof of your identity before responsive record(s) will be released to you.

If you are an agent or intermediary, you must pay the application fee and also, provide proof that you have authority to act for that person (e.g. executorship, guardianship or trusteeship order, power of attorney) before records are released to you. The authorization must be signed (electronic signatures are not acceptable) and dated by that person.
